Are you or your child's teacher think your child may have ADHD, attention-deficit/ hyperactivity disorder? If so, please call your child's home office? We will provide a comprehensive packet of information and a set of forms that need to be completed by both parents and the child’s teachers. After these forms are completed, we will make an intial parent-only appointment for an ADHD consultation with one of our physicians. The information you and your child’s teachers provide will assist your provider in determining if your child has any clinical or diagnostic findings consistent with ADHD.
